At Anytime Auto Glass and Tint, we value our employees and appreciate the hard work you contribute every day. To support your health and well-being, we pay 100% of your Employee-Only Everydaycare Routine Care premium or 50% of your Employee-Only Everydaycare Hospital premium—a contribution of $236.50/mo ($54.58/wk). If you choose Hospital PLUS, that same dollar contribution applies toward your premium. These contributions apply to employee-only coverage. If you add a spouse or children, you pay the additional premium along with any remaining employee premium.

Employer pays $236.50/month toward Hospital or Hospital PLUS; employees pay the remaining premium.

Dental & Vision Insurance

Delta Dental & Vision

Two separate policies — Delta Dental PPO™ and Vision Care.

Anytime Auto Glass and Tint offers separate dental and vision policies through Delta Dental of Arizona. Enroll in either or both; each is billed separately and is 100% employee-paid. Delta Dental PPO™ costs $35.73 per month ($8.25 per week) for employee-only coverage. Preventive care is covered at 100% with no deductible. Basic services are covered at 80% and major services at 50% after the deductible. There is a $50 individual or $150 family deductible and a $2,000 annual maximum per person from January through December. Vision Care costs $7.64 per month ($1.76 per week) for employee-only coverage. It includes a $10 eye exam copay, a $150 frame allowance or $150 contact lens allowance, and a Diabetic Services Rider covered twice yearly with $0 copays.

Supplemental Insurance

Aflac

Five voluntary policies — cash benefits paid directly to you.

Anytime Auto Glass and Tint offers five voluntary supplemental insurance policies through Aflac: Cancer Protection Assurance, 24-Hour Accident, Hospital Choice, Short-Term Disability, and Critical Care & Recovery. Aflac pays cash benefits directly to you to help offset out-of-pocket medical bills, everyday living costs, and lost income when a covered illness, injury, or hospital stay occurs. All policies are 100% employee-paid through weekly payroll deductions with no employer contribution.
